Florida Department of Health in Hardee County

The mission of the Florida Department of Health in Hardee County, Vital Statistics Office is to build and maintain a complete uniform system of accurate vital information which satisfies not only the legal requirements mandated by the State, but also to provide individuals and their families with timely and convenient access to vital records.

A valid Photo ID is required when requesting cause of death. Death Certificates available only from 2009 to current year at this location.

A Death Certificates with the cause of death can be issues to:

  • The registrant's spouse
  • The registrant's parent
  • The registrant's child
  • The registrant's grandchild
  • The registrant's sibling

Death Certificates without the cause of death are open records and available to anyone. 

Fees

  • Fee: $10.00 each
  • Additional Copies: $10.00 each
