Community Health Status

A community health assessment is the systematic collecting and analyzing of data to educate and mobilize communities, develop priorities, garner resources, and plan actions to improve public health.  Through the substantial amounts of data available in Florida Charts, the FDOH-Hardee County can provide the analysis of data for the assessment process.

The comprehensive Community Health Assessment (CHA) was published in January 2023 and the Community Health Improvement Plan (CHIP) was published in March 2023. The Strategic Plan and Performance Management and Quality Improvement Plan were also published in 2023.
