Florida Department of Health in Hardee County

  • Valid Photo ID Required (when requesting cause of death)
  • Fee: $10.00 each
  • Additional Copies: $10.00 each

Issuance (with cause of death) to:

  • the registrant's spouse
  • the registrant's parent
  • the registrant's child
  • the registrant's grandchild
  • the registrant's sibling

*Certificates of Death without the Cause (open records and are available to anyone)

*Death Certificates available only from 2009 to current year at this location.
